I collect the information you choose to send me through this website's contact form, by email, or by phone. This may include your name, email address, phone number, state, scheduling preferences, how you heard about me, and any message you write describing why you are seeking therapy. I use this information only to respond to your inquiry and to schedule a consultation. I do not use it for any other purpose.

I collect this information directly from you.

3. Who I share it with

I share this information only with the service providers that help me operate this website and respond to you:

• Squarespace (website and contact form hosting)

• Google Workspace (email)

• iPlum (phone)

These providers act on my behalf and are required to protect your information. I have no affiliates. I do not sell consumer health data, and I do not share it for advertising.

4. Your consent

I collect and share consumer health data only with your consent, as necessary to provide the service you request, or as required by law. When you submit the contact form, you are asked to give your consent. You may withdraw your consent at any time.

5. Your rights

You have the right to:

• Confirm whether I am collecting, sharing, or selling your consumer health data

• Access your consumer health data, including a list of all third parties and affiliates with whom I have shared it

• Withdraw your consent to my collection and sharing of your consumer health data

• Ask me to delete your consumer health data

To make a request, email tsuki@niutherapy.com. I will respond within the time required by law. If I decline your request, you may appeal by replying to my response. If your appeal is denied, I will tell you how to contact the Washington State Attorney General to submit a complaint.
